Chief Complaint: Blunt Trauma, Airway Instability


Treatment Narrative


On arrival, patient laying supine on front walkway to mall. Care underway by fire crews on scene. Patient is 28yom with injuries secondary to a fall through collapsed stairs during a structure fire. Patient is not conscious with labored breathing. On scene crews instructed to assist ventilations. NPA placed, high flow O2 via NRB and subsequently BVM.

Secondary assessment is unremarkable for external injury. Vitals assessed. Patient found to be tachycardiac and hypotensive, hypoxic. IV access by Chief Santiago in the right external jugular.

Patient's respiratory status critical, patient deemed to require invasive airway stabilization. Visualization of the airway performed by direct laryngoscopy. Severe airway inflammation observed in oropharynx and proximal to vocal folds. Unable to pass bougie device through to the trachea. Passive oxygenation resumed.

Patient administered 10mg midazolam IVP for further sedation. Crichoid membrane digitally identified. Site cleaned with iodine. 1/2 inch incision made through the crichoid membrane and into the trachea, cleared with finger. 8.5 ET tube inserted into trachea, secured. BVM ventilations initiated with good ETCO2 waveform and positive improvement in SPO2 readings. Pt rapidly extricated to Medic 1, Code 3 transport to CMM. Fluid bolus en route with positive improvement in vital signs.
